District Nurses
District Nurses are qualified nurses who have undertaken a specialist qualification in Community Health. They manage a team of qualified and unqualified nurses to deliver nursing care to patients in their homes. The service is provided for people who are housebound and will assess, plan and provide the most appropriate high standard nursing care. It is a 24-hour responsive service that promotes patient independence through teaching and supporting the patient and their carers. They work in partnership with other members of the Health Care Team and local agencies to maximise the opportunity for patients to live at home.
