It is important that we have up to date records of your personal details, home address and telephone numbers, and e-mail addresses. If you change your name, address or telephone number please let our receptionists know at the earliest opportunity, by e-mail is acceptable. If you move outside the practice area you will have to find a doctor in your new area.
